9th Grade Course Request Form
(for current 8th grade students registering for 9th grade)
Tips for 9th Grade:
- If you choose to take Secondary Math 1 Honors, you will need to choose an extra elective class. It is strongly recommended that you enroll in Knight Academy study hall class for a full year to support you because the work load will be heavy in the single-blocked math class.
- If you choose to take an AP class, you must also take AP Research for the entire year. AP classes are college-level classes that provide an opportunity to earn college credit if you pass the test at the end of the year. The work load for AP classes is high.
- The semester-long geography class covers an entire year of material in a semester, and is therefore considered an advanced class. The work load is high. If you choose to take the semester geography class, you will need to choose an extra elective class for 2nd semester.
- Biology is an advanced course if taken in 9th grade. The work load is high. If you choose to take biology in 9th grade, you will need to take chemistry or physics in high school to meet the state science requirement for graduation. Both chemistry and physics require higher level math skills. If you take earth science next year, you will be able to choose biology, chemistry, or physics as your second foundation science class at the high school.

NOTE: To qualify to take the accelerated 7/8 math class, students must take the CogAT assessment or complete a portfolio and score at or above the 90th percentile.
Step 3: Fill Out Course Selections on MyDSD
This year you can complete your own course request on myDSD.
- Completing the paper course request form first will guide you through this step and help you avoid mistakes. Completed course request forms will be submitted to school counselors when counselors visit classrooms the week of February 7-11.
- If possible, please try to complete Step 3 before February 7th when counselors will start visiting classes.
- The course request screen will be open for you in myDSD from Monday, January 31st through Friday, February 11th.
- Until February 11th, you will be able to return to make changes if needed after your initial submission of the online form.
- The course request screen must be accessed through the student's myDSD account to add classes or make changes. The parent account is currently a view-only screen.
- Please note: Select your registration group to preload core and required classes. If you would like to take an honors or advanced class for any of the core subjects, remove the preloaded course and add the honors/advanced class.
- Knight Academy study hall and LDS Seminary are found under "Miscellaneous Electives."
If you have questions or are confused by Step 3, don't worry! Just bring your completed paper course request form to CCA or English 8 the week of February 7-11, and school counselors will answer your questions and help you with this step.
Step 4: Finalize Course Selection
If possible, please enter your course selections in myDSD (Step 3) before the week of February 7-11 when school counselors will visit CCA and English 8 classes to review your myDSD course requests, answer questions, and help you finalize your course selections. Please bring your paper course request form to class with you on the days indicated below to help facilitate this registration finalization process.
Incoming 9th Grade Students:
School counselors will visit your English 8 class to help you double check and finalize your online course selections on Monday and Tuesday, February 7 and 8, and they will collect your completed paper course request form at that time. If possible, please enter your course selections on myDSD (Step 3) before February 7th.
Incoming 8th Grade Students:
School counselors will visit your CCA class to help you double check and finalize your online course selections on the following dates, and they will collect completed paper course request forms at that time. If possible, please enter your course selections on myDSD (Step 3) before these dates:
Mr. Balling's classes - Monday, February 7 and Tuesday, February 8
Mrs. Beebe's classes - Wednesday, February 9 and Thursday, February 10
Mrs. Burton's classes - Thursday, February 10 and Friday, February 11
Incoming 7th Grade Students:
Registration packets will be distributed to all elementary schools before February 14th so you have time to review the information before the 7th Grade Registration Open House at KJH on February 17th:

5:00-5:45 Windridge Elementary and Kaysville Elementary
6:00-6:45 Columbia Elementary and other schools
School counselors and administrators from KJH will visit your elementary school on the following dates to meet with you, answer questions, and help with the registration process.
Thursday, February 24 - Windridge Elementary
Friday, February 25 - Burton Elementary
Friday, March 4 - Columbia Elementary
Monday, March 7 - Kaysville Elementary
Please bring your completed paper course request form to your individual registration meeting at your elementary school in February/March. Your registration packet will include your individual meeting time.
*** If you are absent or unable to return your course request form during class, please return it to the Counseling Office no later than Friday, February 11.
